Welsh Water Sensitive Customer form

To provide the correct and most up to date emergency contact details, any site-specific health & safety information and storage facilities on site. Typically, we use this information to contact you during unplanned events, which can occur outside normal business hours.

Application for a leakage allowance

This form to be completed to make an application when a underground leak has occurred on your private network. Any allowance paid is based on the sewerage element of your bill only. Documentary evidence of a successful repair will be required.

Direct Debit Mandate

To provide bank details and instructions for a direct debit to be set up on your account.

Third party letter of authority template

A template letter to be used when requesting authority for a third-party company to access account information or to act on your behalf.

Your Wastewater

Non-Return to Sewers Allowance

To be completed if you believe that not all the water consumption recorded by the meter returns to the sewer network. A 5% allowance is given as standard, any application would consider a higher allowance (e.g. water consumed through the meter is used in a product or evaporation occurs in production processes).

Surface water drainage rebate form

This form should be used if you believe that the drainage coming off your buildings are not returning rainwater to the sewerage network. Plans/maps should be provided with this form.

Trade effluent assessment form

This form is used to provide up to date details of your domestic sewerage. We use this form as part of an assessment process for your trade effluent account. These details are used to ensure that your bills are up to date and accurate.

Trade effluent discharge application

To be completed if you would like to apply for a new trade effluent consent.

Industrial Tariff Application

If you believe you have a single site where the consumption is above 50 megalitres per annum and you wish to apply for an industrial tariff banding, please complete this form.

Access to asset form

Form to be completed by the customer (or their contractor) to request permission to access a Dwr Cymru Welsh Water asset for any works (e.g. to install a private data logger).
